Treatment of onychomycosis

Onychomycosis—a fungal infection of the nail plate, nail bed, or surrounding tissue—is one of the most persistent and challenging dermatological conditions affecting the foot. Accounting for roughly half of all nail disorders, it affects up to 10% of the general population and up to 20% of adults over the age of 60. Beyond its aesthetic impact, onychomycosis can cause physical discomfort, impairment in walking, and heightened risks for secondary bacterial infections, particularly in individuals with diabetes or peripheral vascular disease. Treating onychomycosis requires a nuanced, multi-faceted approach tailored to the severity of the infection, the patient’s overall health, and the specific fungal pathogen involved.

Etiology and Diagnostic Essentials

Successful treatment begins with accurate diagnosis. Onychomycosis is primarily caused by dermatophytes—most notably Trichophyton rubrum and Trichophyton mentagrophytes—which account for roughly 80% to 90% of cases. Non-dermatophyte molds (such as Aspergillus or Fusarium species) and yeasts (primarily Candida albicans) cause the remainder.

Because non-fungal conditions like psoriasis, lichen planus, trauma, and aging can mimic the yellowing, thickening, and subungual hyperkeratosis typical of fungal infections, visual diagnosis alone is insufficient. Modern clinical guidelines strongly recommend laboratory confirmation before initiating systemic treatment. Standard diagnostic methods include:

  • Potassium Hydroxide (KOH) Prep: Provides a rapid, cost-effective screening for fungal hyphae.
  • Fungal Culture: Identifies the exact pathogen species to guide targeted therapy.
  • Periodic Acid-Schiff (PAS) Staining: Offers high sensitivity through histopathological examination of nail clippings.
  • Polymerase Chain Reaction (PCR): Rapidly detects fungal DNA with high accuracy, though it remains less widely accessible.

Topical Antifungal Therapies

Topical monotherapy is generally reserved for mild-to-moderate cases of distal lateral subungual onychomycosis (DLSO) that involve less than 50% of the nail plate without involvement of the matrix (the nail root).

Topical treatments face a significant mechanical obstacle: penetrating the hard, keratinized structure of the human toenail. Modern formulations utilize specialized lacquers and solution vehicles designed to enhance penetration into the underlying nail bed.

Terbinafine

Terbinafine (an allylamine) is widely considered the first-line oral agent for dermatophyte-induced onychomycosis due to its fungicidal action. It inhibits squalene epoxidase, an enzyme crucial for ergosterol synthesis, leading to squalene accumulation and fungal cell lysis.

  • Standard Dosage: 250 mg daily for 12 weeks for toenails.
  • Efficacy: Mycological cure rates range between 70% and 80%, with clinical cure rates around 60%.
  • Safety Considerations: Terbinafine undergoes hepatic metabolism. Baseline and mid-treatment liver function tests (LFTs) are recommended due to rare risks of drug-induced liver injury. Common mild side effects include gastrointestinal distress and transient taste disturbance (dysgeusia).

Itraconazole

Itraconazole (a triazole) provides broad-spectrum fungistatic coverage, making it particularly useful when non-dermatophyte molds or yeast species are suspected. It inhibits the cytochrome P450-dependent enzyme 14$\alpha$-demethylase.

  • Dosing Options: Can be administered as continuous daily therapy (200 mg/day for 12 weeks) or pulse therapy (400 mg daily for one week per month, repeated for 3–4 pulses).
  • Safety Considerations: Itraconazole carries a heavy potential for drug-drug interactions because it strongly inhibits the CYP3A4 enzyme. It is contraindicated in patients with ventricular dysfunction or heart failure due to potential negative inotropic effects.

Device-Based and Surgical Interventions

When pharmacotherapy is contraindicated—such as in patients with liver disease, polypharmacy risks, or severe drug allergies—physical and mechanical modalities offer alternative avenues.

  • Mechanical Debridement: Trimming, thinning, and grinding down thickened nails reduces the fungal burden and relieves pressure pain. Debridement also increases the surface permeability of the nail plate, significantly improving the penetration and efficacy of concurrent topical treatments.
  • Laser Therapy: Devices using 1064 nm Nd:YAG lasers aim to destroy fungal tissue through selective photothermolysis without damaging surrounding skin. While FDA-cleared for the “temporary increase of clear nail,” laser monotherapy generally yields lower long-term mycological cure rates compared to systemic oral agents and is often used as an adjunct.
  • Chemical or Surgical Avulsion: In severe, refractory cases or painful localized dystrophy, partial or complete nail removal using high-concentration urea pastes (40%) or surgical avulsion can clear hyperkeratotic tissue and allow direct application of topical therapies to the unroofed nail bed.

Prophylaxis and Long-Term Recurrence Management

One of the most frustrating aspects of onychomycosis is its high recurrence rate, estimated between 20% and 50%. Fungal spores survive easily in dark, moist environments like shoes and communal floors, making long-term prevention as vital as the initial treatment phase.

Key Steps for Recurrence Prevention:

  • Foot Hygiene: Keep feet dry and clean; change moisture-wicking socks daily.
  • Footwear Management: Rotate shoes to allow at least 24 hours of drying time; treat the inside of footwear with antifungal sprays or ultraviolet (UV) shoe sanitizers.
  • Prophylactic Topicals: Apply over-the-counter antifungal powders or sprays to the feet and interdigital spaces, particularly if tinea pedis (“athlete’s foot”) is present.
  • Trauma Reduction: Wear properly fitted shoes with adequate toe-box space to prevent micro-trauma, which creates portals of entry for fungal pathogens.

Ultimately, effective management of onychomycosis demands patience and clear communication. Because toenails grow slowly—taking anywhere from 12 to 18 months to fully replace themselves—patients must understand that visual clearing of the nail takes time, even long after the underlying fungus has been successfully eradicated. Combining accurate laboratory confirmation, tailored systemic or topical regimens, and diligent prophylactic habits offers the highest probability of lasting success.
